Job Title: Infection Prevention Nurse LVN

Job Summary:
Linwood Meadows Care Center, a reputable Skilled Nursing Facility, is seeking a dedicated and detail-oriented Infection Prevention Nurse LVN to join our team. In this role, you will be instrumental in developing, implementing, and monitoring infection prevention and control programs to ensure the health and safety of our residents and staff. You will work collaboratively with the interdisciplinary team to uphold the highest standards of care while complying with state, federal, and facility guidelines.

Key Responsibilities:

• Infection Control Program Management:
  
  • Develop, implement, and periodically review infection prevention policies and procedures in accordance with CDC, CMS, CalOSHA, and other applicable guidelines.
  • Monitor and track infection rates and potential outbreak indicators, initiating timely interventions when necessary.
• Surveillance & Reporting:
  
  • Conduct regular audits and environmental assessments to ensure compliance with infection control protocols.
  • Collect, analyze, and report data on infection trends, preparing detailed reports for facility leadership and regulatory bodies.
• Staff Education & Training:
  
  • Provide ongoing education and training to clinical and support staff on best practices in infection prevention and control.
  • Act as a resource and mentor on infection prevention matters, ensuring staff remain current with evolving practices and guidelines.
• Collaboration & Communication:
  
  • Work closely with the nursing team, physicians, and other healthcare professionals to identify potential infection risks and develop appropriate responses.
  • Participate in facility-wide committees and meetings to contribute to quality improvement initiatives and emergency preparedness planning.
• Outbreak Management:
  
  • Assist in the development and execution of outbreak response plans.
  • Coordinate efforts during infectious disease outbreaks, ensuring prompt implementation of control measures and communication with relevant authorities.
• Documentation & Compliance:
  
  • Maintain accurate records of infection control activities, audits, and training sessions.
  • Ensure all infection prevention efforts meet regulatory and accreditation standards.

Qualifications:

• Licensure & Certification:
  
  • Must hold a valid California LVN (Licensed Vocational Nurse) license.
  • Current Basic Life Support (BLS) certification is required.
  • Additional certifications in infection prevention or related fields are a plus.
• Experience:
  
  • Prior experience in a Skilled Nursing Facility or similar healthcare setting is preferred.
  • Demonstrated knowledge of infection prevention protocols, epidemiology, and outbreak management.

  • Strong analytical skills with the ability to interpret data and implement effective interventions.
  • Excellent communication and interpersonal skills, with a collaborative approach to teamwork.
  • Detail-oriented and highly organized, with the ability to manage multiple priorities.

Working Conditions:

• This role may require exposure to infectious agents and adherence to safety protocols, including the use of personal protective equipment (PPE).
• Flexibility to work as needed to support outbreak management or special projects is beneficial.
