Division/Department/Program: Mental Health (MH) Supportive Housing
The Residential Wellness Property Coordinator is responsible for overall management of GHFA PSH sites under Mental Health Supportive Housing, primarily in Rockdale, Newton & Gwinnett Counties. Ensures efficient and effective operational performance by working closely with the individuals served, vendors, internal & external stakeholders and Mental Health Supportive Housing team members. Provides administrative leadership, day-to-day oversight of properties, and supports the Program Manager. Responds to questions, requests, or needs based on specific knowledge of the program and its operation. Ensure compliance, efficiency and client satisfaction; problem solving.
Under direct supervision of Mental Health Residential Property Management Program Manager, performs a variety of administrative, care coordination and resource linkage for individuals served in Mental Health Residential services.
Duties & Responsibilities:
Coordinates and assists with screening referrals to ensure applicants meet eligibility criteria
Implements harm reduction philosophy in care coordination to ensure swift access to PSH with low barriers
Provides ongoing case management to support residents in maintaining stable housing and moving towards individualized, meaningful goals targeted at assisting residents in obtaining and/or maintaining independence
Assist with coordination of moves and transitions based on clinical need and direction from treatment team; support residential team by maximizing occupancy and making transitions as quick as possible
Monitors resident activities to ensure adherence to Lease Agreements, promptly responds to lease violations to maintain community safety and support residents in maintaining housing
Maintain residents files and conducts periodic chart audit to ensure compliance with regulatory agencies (e.g. DCA, CARF, Integral, HUD)
Assist clients in securing social security benefits through coordination with Behavioral Health Center.
Develops and maintains partnerships with community apartment complexes to secure units within in timely fashion
Maintains relationship with community partners instrumental to business operations and integral to helping residents prosper and thrive in PSH
Secures furniture and utility for residential apartments and housing units; secure apartment supplies; initial setup (create list) and ongoing ordering/delivery of needed supplies and equipment.
Keeps detailed expense records to ensure maximum utilization of grant funds
Schedule and coordinate GHFA Inspections for new setup and inspection renewals.
Coordinates with the business office and facilities maintenance department regarding repairs in apartments and housing units.
Provides invoices to the business office within the expected timeframes to ensure timely reimbursements from the Grantor. Maintain system for tracking payments.
Complete grant renewal applications as needed

Pull reports in HMIS & Emphasys
Conduct quarterly internal inspections/unit visits to ensure all housing units meet housing quality standards
Programming Specific Duties:
Must be flexible to work with clients and vendors in the community.
Travel required throughout surrounding counties
Minimum Qualifications:
High school diploma/GED and three (3) years of job-related experience providing social or direct care services to groups of special needs individuals in a human service setting, which includes one (1) year experience as a lead worker, team leader, or supervisory role; or one (1) year of experience at the lower-level Client Support Worker 3 (SSS012) or position equivalent.

Experience working with individuals receiving PSH residential services
Familiar with compliance and regulatory standards of DCA and HUD
Requirements/Competencies:
Must be able to lift 20 pounds.
Requires long periods of sitting/standing.
Must have valid Georgia driver's license and Motor Vehicle Record in accordance with company policy.
Candidates for selection must pass a criminal background check (including fingerprinting).
Must pass a pre-employment drug screen and subject to random drug screens.
Any combination of training and experience which would have enabled the applicant to acquire the necessary knowledge, skills and abilities.
Note: Some positions may require a valid driver's license.
