View Point Health is a state agency of Georgia that provides individual and group counseling, medication management, recovery and crisis stabilization services for behavioral health and developmental disabilities. Our facility is a 10-bed crisis stabilization unit located on 7 beautiful acres of land in Conyers, Georgia. We serve adolescents ages 10-14 on the Autism Spectrum and provide a strong multidisciplinary team approach incorporating Applied Behavior Analysis, mental health clinical services, and medication management.
Under general supervision, provides basic patient care and works with patients/clients to develop daily living skills. May take vital signs.
Duties & Responsibilities:
Provides basic patient care and works with patients/clients to develop daily living skills. May take vital signs.Monitors general client activity to observe and report behavior/progress of clients admitted to a community residential crisis stabilization center for treatment of acute mental illness or behavioral health disorders.Maintains awareness of clients behavior and physical whereabouts to ensure their safety. Observes and becomes familiar with each clients behavior and disability. Monitors clients for detection of any behavior which could be injurious to self or others, or which is so unusual or atypical that immediate attention of nursing staff is warranted. When necessary for the physical safety of the client, uses the least restrictive, agency-approved behavioral or physical intervention technique appropriate for the situation. Secures assistance from and/or helps other staff in handling acute behavioral crisis situations.Provides personal care and basic nursing procedures to clients, including feeding, personal hygiene and grooming as necessary.Uses all appropriate infection control and other safety measures.Attends client specific meetings and trainings, including trainings on clients Behavior Support PlansAccurately collects data (paper and/or electronically) on target and replacement behaviors specific to the clients Behavior Support Plan.Reports changes in client condition to supervisor and Registered NurseAssists with Activities of Daily Living (toileting, feeding, hygiene, etc.), to meet the needs of the client.Provides important information regarding the client to facilitate appropriate case management and collaboration.Follows established procedures for clients on special precautions, such as suicidal watch, close observation, escape precautions, seclusion and/or restraints.Interacts with clients positively and respectfully. Maintains clean and organized condition to assure a safe, secure, and attractive environment for clients and staff. Provides instruction and support to each client in maintaining his/her personal living space in a clean and orderly manner.Assists in serving meals, with laundry.Obtains vital signs. Records clearly, concisely and accurately all client care data and other documentation in medical records according to agency protocols.Assembles new chart packets.Provide transportation of clients when needed.Assists with completion of daily census records.Helps with admission intake process.Conducts skills groups, recreational activity groups and community meetings with clients. Individual Job Description:
Assist with the facilitation of DBT skills groups for the appropriate number of times per week.Facilitate Activity groups for the appropriate number of times per week.Monitor clients during Breakfast, Lunch, and DinnerComplete admissions process for new arrivals.Prepare clients belongings upon discharge.Monitor and assist clients during the hygiene routine.Assist clients with scheduled phone calls. Document those phone calls.Conduct safety checks for new admissions as well as current clients if necessary.Assist with manual holds/restraints.Monitor and assist during unit visitation.Organize the unit.Conduct room sweeps for contraband or dangerous items.Complete daily client observations sheets for assigned client/s.Monitor group for appropriate client behaviors.Assist with Receive and Evaluate clients.Complete the daily 24-hour report to document the clients behaviors during the shift.Sit with 1:1 client all day and ensure that they are never more than an arms-length away or without supervision.Transport clients if needed.Attend mandatory monthly meetings.Attend Required Trainings for professional development.Additional duties as assigned by supervisor.All applicants must be capable of implementing all of the physical behavior management/ manual techniques (CPI or MANDT) for employment.
Minimum Qualifications:
High School Diploma or GED AND Six months of experience in a related field
Preferred Qualifications:
At least six months of experience in a related fieldExperience serving individuals with Autism Spectrum Disorder preferredDirect Care ExperienceExperience with individuals being served in a psychiatric hospital, residential, or group home setting preferred.Experience and/or knowledge of DBT is preferred.Experience of at least one year working with Dual Diagnosed (Substance Abuse/Mental Health) and or residential settingInterventions, Diagnosing, Treatment PlansExperience with Care Logic or other EMRIs comfortable and proficient with collaborative & concurrent documentation.Experience in trauma-informed care, or trauma related interventions and skills
Requirements/Competencies:
This position is on-site and does not accommodate a remote schedule or working from home.Must be able to lift 20 pounds.Requires long periods of sitting/standing.Must have valid Georgia drivers license and Motor Vehicle Record in accordance with company policy.Candidates for selection must pass a criminal background check (including fingerprinting).Must pass a pre-employment drug screen and subject to random drug screens.Any combination of training and experience, which would have enabled the applicant to acquire the necessary knowledge, skills and abilities Note: Some positions may require a valid driver's license.Other duties as assigned.
