Care Coordination Supervisor (Healthcare)
Job Summary
HCAM offers diverse connection and coordination services for individuals and families across Maryland through various programs. HCAM's MATCH Program (Making All the Children Healthy) provides coordination, education, and advocacy to ensure Baltimore City youth who are in the foster system or reuniting with their families receive the physical and behavioral health care they need through age 21.
The Care Coordinator Supervisor will serve as a part of MATCH's team by providing direct supervision of the Care Coordinator Associates to ensure that standards of practice are met in working with individual clients and their families.
Duties And Responsibilities:
Requirements & Experience:
Bachelor's degree required; Master's degree preferred.
A minimum of 3 years of management or relevant leadership experience
Knowledge of the foster care system and health field preferred
Relevant clinical experience providing services to families and children, preferably in a public health/community setting.
Excellent planning and organizational skills
Well-developed interpersonal & communication skills
Professional appearance and demeanor
Computer literate specifically MS Word, Excel, Outlook, and Gmail
